From my experience it was always a guaranteed drop, perhaps the show percentages are messed up because of master looter.

The percentage is lower than 100% because it can only drop for 1 player once(as this is not a repeatable quest) so that means when they kill the dragon again it wont drop for them.

For those of you who might be having trouble 2-manning or 3-manning these bosses:

A good way to rescue party members who have been slept by the dragon is to have the tank move the dragon so his tail is against those players who are asleep. Like all dragons, the tail will eventually toss those players back and out of the sleeping cloud, allowing them to return to the fight.
