<name>, you are a dependable <race>. Can I count on you for another task?

Some time ago I was surveying the cave to the north for minerals, and I left my favorite pick behind. When I later returned to retrieve it I found the cave was filled with vicious beasts! Will you go into the cave, the Burning Blade Coven, and get my pick?

I left it in a chamber with waterfalls. My pick has a spell on it so you can see it in the dark, so you won't have to worry about finding it... just what's guarding it!

It's at the source of the waterfall (43, 54) that's in the main chamber. Find the waterfall and follow it back to the source.

Go into the cave to the north where the vile familiars are walking around.

When you come to the first split, you can go left, straight, or right. Go STRAIGHT.

Now walk to the RIGHT, there should be a stream of water on your left. Follow that stream until it stops at a waterfall. The pick is large and glowing, is right next to the water, and is behind a column of rock.

If you still can't find it, the pick is in the middle of the entire cave (LOOKING FROM YOUR MINI-MAP, ZOOMED OUT MAX).

There you go!

Its in the middle of the cave with about 3 vile familiars and 1 felstalker walking near it.I'd recommend killing them first and then take the pick,rogues can simply stealth through the cave to get to it although it will deactivate your stealth when you loot it and the nearest enemy(enemies) will attack you.Try do it with Burning Blade Medallion so you don't have to enter this annoying cave twice!Happy Questing=) Exact coords [43.7, 53.8]
Edit:As of 3.3 the mobs in this cave are no longer aggresive so you can just walk in and take it...=/

Such a trivial quest now, before 3.3 the Vile Familiars and Felstalkers were all hostile and would aggro when you were within close proximity of them, now they're neutral. It's as simple as walking in, taking the pick, and walking out now.

As you enter the cave, keep going through the opening you see straight ahead. There, next to the water fall, should be Thazz'ril's pick. Loot it and move on with your adventures in Azeroth.
