Only the initial turn-in will give you a choice of one of the three quest rewards. Subsequent turn-in's of the Head of Onyxia will only trigger the buff of the Rallying Call of the Dragonslayer... so chose wisely!

The buff is level based. If your 63+ you won't get it.

If you are above level 63 leave it in your bank for a while and the next time a horde raid is in Sw turn in the quest. That will give them a challenge =D

I turned in this quest tonight (first time for this character), and I saw no shout message from the NPC, and I don't believe the buff happened either. My friend was on his level 16 priest in the Auction house, and he said nothing happened. So I guess he only shouts before the buff.
